Tracking Safety Incident in Contractor Foreman is critical to the continued success of your company. Contractor Foreman gives you the tools to track Incidents and create and track the Federally Required OSHA 300 Log. Without the proper tracking of Incidents and Safety related items, you can be exposed to large OSHA fines and higher insurance costs. Incidents is one of the tools Contractor Foreman provides you with.

To create a new Incident, go to main menu and click on Incidents then click on either the small New button to the right of Incidents or click on larger blue button Create New Incident in middle pane. Required fields are marked with and Asterisk (*.)

Project: Select the appropriate project from the drop down. It also has defaults for Office, Warehouse, Driving and other as Incidents can happen anywhere.

Incident Date and Time: Fill out the date and time the Incident happened.

Type: Select the type of incident that occurred. Accident, Near Miss or Death.

Incident Description: Type a detailed description of what happened. You can add steps taken to correct later.

People Involved: Use the blue button to select who was involved. This is NOT where you put witnesses.

People Notified: Use the blue button to select who was notified of the incident.

When Notified: Use the blue button to show when people (usually management) were notified.

Witnesses: Use the blue button to select who witnessed the incident.

Incident Images: Select and photos taken of the incident.

Incident #: System will auto generate and auto populate this field.

Any Injuries: Most times an incident will not result in injuries. It is still important to document the incident. Select Yes or No box. If you select Yes a text field will open below where you need to fill out a detailed description of the injury.

Steps Taken to Correct: Write a detailed description of the steps that were taken to correct why the incident happened.

OSHA Violations?: Select Yes or No. If you select yes a text box titled Describe OSHA Violation will open. Use this to describe the violation.

Note: This is a place to enter any other general notes.

IMPORTANT: If your company is required to complete OSHA 300 logs, Contractor Foreman makes it easy to add Employees to an incident. Each Employee will have their own OSHA 300 line-item per OSHA requirements. Follow these steps below:

Click Add Employee button. A pop up will appear, fill out all fields.

Select Employee: Select Employee from drop down box.

Title: Add the injured employees title.

Where the event Occurred: Write a description of where the injury took place.

Classification: Select the appropriate item from the drop down. Death, Days away from Work, Job transfer or Restriction, or Other Recordable Case.

Days Away from Work: When known enter the amount of days the employee was off work.

Days on Job Transfer or Restriction: When known enter the amount of days the employee was on transfer or restriction.

Injury Illness or Type: Select from one of the items in the drop down.

Select Save to save the record. This will then take you back to the Main Incident Screen.

To save record click on the Submit Button

To Edit the record, click on the green pencil icon in the upper right of screen.
