Keeping accurate OSHA records is an essential part of workplace safety and compliance. Creating an OSHA report for an employee allows you to document any work-related injury, illness, or event that meets OSHA’s recordable criteria. This ensures your company remains compliant, transparent, and ready for audits or inspections.

Why It’s Important

Accurate OSHA reports help you:

  • Maintain legal compliance with OSHA’s injury and illness recordkeeping requirements (29 CFR 1904).
  • Track workplace safety trends to identify and correct recurring hazards.
  • Provide evidence of due diligence in case of insurance or inspection reviews.
  • Promote a culture of accountability and safety awareness within your organization.

How to Create an OSHA Report in Contractor Foreman

  • Go to the Incident Module
Navigate to the Incident module from your main dashboard.

  • Select or Create the Incident Record
    • If the event has already been logged, open that Incident Record.
    • If not, click the “+ Incident” button to create a new one and enter all required information such as the type, location, date, time, and description.

  • Click on the OSHA 300 Log Button
Inside the incident record, locate and click the “OSHA 300 Log” button. This will open a new pop-up window where you can create the OSHA entry.

  • Enter the Required Employee and Event Details
    Fill out the following fields:
    • Employee name
    • Event title or brief description of the incident
    • Location (project or site where it occurred)
    • Classification (e.g., days away from work, job transfer, or other recordable case)
    • Injury or illness type (e.g., cut, sprain, burn, respiratory issue, etc.)
  • Save the Report
Click Save once all required fields are complete. The employee and event will now be recorded in the company’s OSHA 300 Log.
  • Generate or Print the OSHA Report
If needed, you can click the three-dot menu (⋮) in the incident record and choose View / Email PDF to download or print a copy of the OSHA report. This can be shared with HR, management, or OSHA inspectors as required.

Additional Best Practices

  • Always review the information before submitting to ensure accuracy.
  • Attach supporting files (photos, witness statements, medical reports) under the Files tab in the Incident Record.
  • Use the Notes section for internal communication between safety or management teams.
  • Conduct a root cause analysis and implement corrective actions to prevent recurrence.

In Summary

To create an OSHA report for any employee, simply open or create an incident, use the OSHA 300 Log button, and enter the event details. Once saved, your record will be securely logged, linked to the employee, and ready for compliance reporting. Keeping these reports up to date ensures both regulatory compliance and a proactive safety culture on every jobsite.
